  5. PUB 408 Specifications 108.01 SUBLETTING OR ASSIGNMENT OF CONTRACTS (a) General. Do not sublet, sell, transfer, assign, or otherwise dispose of the contract or contracts or any portion or rights, title, or interest, without the written consent of the Representative… (e) Subcontract Approval. Do not allow any work on the project that is assigned to others (including services) until a copy of the executed subcontract…has been reviewed and acknowledged by the Department… The ECMS subcontractor request must be approved prior to the start of the subcontractor’s work.

Sub on Prime’s Payroll • Requirements • Union Hall Labor Solicitation Documentation (if applicable) • Employment Application On-Site • W-4 Form • I-9 Form • Fringe Benefit Letter • Copy of Cancelled Check to Employee • Equipment Rental Agreement AND Copy of Cancelled Check for Equipment Rental (if applicable)

  20. 2nd Tier Subcontractor • Requirements • Prime Submits 1st Tier Request • 1st Tier must be Approved • 1st Tier must Self-Perform at least 50% of Work • 1st Tier must be Prequalified to “Oversee” 2nd Tier • Prime Submits 2nd Tier Request with 1st Tier as “Overseer” • 2nd Tier must be Prequalified • 2nd Tier must be Approved • 2nd Tier must Self-Perform 100% of Work
