The case for change… Workforce pressures including recruitment and retention problems The number of people with multiple long term conditions set to grow from 1.9 to 2.9 million from 2008 to 2018 Inequalities in access & quality of primary care 76% of patients rate overall experience of making an appointment as good Growing dissatisfaction with access to services
Our conversations so far has led to priorities… • Patients feel supported and empowered to manage their own conditions • Professionals have access to health records to give more holistic integrated care • Appropriate appointments for patients with complex conditions to support self management of their care • Practices work closer together in Bracknell and Ascot • Work closely through co-commissioning with NHS England to improve premises, workforce, prevention of ill health and improving quality • Practice delivering together minor surgery and arrhythmia services
The story so far… • Innovation projects agreed around: • Self care • Pre diabetes schemes • Clinical commissioning group committed funds to support practices coming together to improve quality and sustain the services • Practices have supported the opportunity for consistent clinical audits, medicines optimisation analysis and sharing of clinical and non clinical protocols • Development of a model for primary care nursing to address the needs of our population • Patient experience shaping the priorities for areas for improvement, working with Healthwatch
The single biggest challenge to General Practice is the need to shift from treating episodic illness to working in partnership with patients and other providers to improve health, and treat people in the community more efficient
