Our Success Factors • Designated staff to do abstraction (FTE allocation) • One abstractor does SCIP • One abstractor does AMI, CHF and pneumonia. • Abstractors are Registered Nurses • Data Dictionary is used during reviews
Our Success Factors • Frequent use of Q-Net Quest (queries printed) • Use your Resource people • Vendor (RWHC) • Metastar (Candi Davis). • Other abstractors if available
Our Success Factors • Electronic charting. • Standardized key phrases for staff charting • Data abstracted at face value. • No gray areas accepted as a positive qualifier.
Our Success Factors • Use of Web based data collection thru vendor. (We use two computers at the time of data entry-one for the web based data entry and one for reviewing our electronic charts) • A data collection sheet is completed with notes made prior to data entry (personal preference by one of our abstractors) • Feedback from validation is noted with changes made if needed
Keeping Current • Periodic Review of Q-Net Website (updates, release notes, etc.) • Alert to e-mails from Q-Net • Participation in MetaStar Webinars • Participation in Vendor Teleconferences
Training • Emphasize and familiarize yourself with each new DATA DICTIONARY. • Print out a paper copy to always have as a quick reference • Train individuals to research out their questions, using Q-Net • Learn how to submit questions • Allow adequate time for the questions to be researched and sent back to you
Training • For new Abstractors • Establish a mentoring program • Provide 1:1 instructions • Always use the Data Dictionary (whether a new abstractor or a seasoned one) • Have another abstractor review work until accuracy and comfort level has been established.
Lessons Learned . . . • Do not do all of the charts the week they are due!!! (Causes heartburn, headache, ulcers and inaccurate data collection- we know from experience) • Allow adequate time for data abstraction so if questions they can be researched. • Abstract the chart at face value. • Compare verbiage from the Data Dictionary with the chart. If you question the terminology, consult the QNet.
