  4. CVD Prevention in the Community • Formed Physical Activity group based on needs of each population • Day, Time, Location, Type of Activity, Childcare, Transportation • Funding • Recruiting/Promotion • Special Activities • Evaluation

  5. Lessons Learned • Flexibility (Location, type of activity) • Climate • Health/Social Benefits vs. Incentives • Recruitment • Sustainability

  6. Successes • Expansion of the program • Bilingual Leaders • CHWs as resources for new physical activity groups • Blood pressures decreased • Catalyst for other behavior changes (dental, depression, diabetes, lead) • Social Support
