Beyond the Collaborative: Our Next Aim Statements (Draft) • 95% of VIHA patients will attend the Arthroplasty Care Clinic (better prep of patient (CDM)?) • 100% of VIHA arthroplasty patients will follow the VIHA Hip/Knee pathway • 100% of patients island wide will follow the new care map • 100% of VIHA therapists will use outcomes from the Milestones document to guide practice/service delivery • 95% of patients will report that pain is well controlled 2 days post D/C
