  1. Federal Aviation Administration Lessons Learned Update Presented to: COMSTAC By: Michael S. Kelly, Chief Engineer Date: 9 October 2012

Background • During October 2011 meeting, COMSTAC developed the following finding: “COMSTAC finds that it would be beneficial for the FAA to develop a process for disclosing to the industry pertinent data from reported safety critical anomalies, mishaps, incidents, and precursors where relevant to current and future operations. Such a process needs to protect proprietary data and comply with relevant export control policies whiles still fostering the continuous safety improvement of the industry” • Subsequently, the House Subcommittee on Space and Aeronautics asked AST: “Do you have a model in mind for a system to exchange lessons learned data between government and industry?”

Response to May COMSTAC • Reminded community of what we currently have: Commercial Space Transportation Lessons Learned Website • Briefed work on internal lessons learned database for safety trend analysis • Adaptation of AST’s STAR database of > 5,600 orbital launches worldwide • Discussed the primary sticking point of sharing lessons learned: proprietary information protection

Update • Internal discussions and one-on-one discussions with industry representatives on proprietary information protection • No progress made • Jim Van Laak on detail to National Institute of Aerospace • Intent is to help create a safety culture in the new commercial space industry • Will be able to engage industry without the formality resulting from direct association with FAA • Will, among other things, encourage data sharing within industry • Will help industry decide what data should be made public on FAA CSTLLS website • Office of the Chief Engineer will work closely with Jim on information-sharing issues • Will update COMSTAC in May 2013

Update -- Continued • STAR Database has been modified significantly, adding fields useful for trend identification • AST investigating whether STAR database can be made available to industry through our website • All information in STAR is from open sources – there is no proprietary information to protect • Need to see if there are any ITAR or other issues • Should have answers by May 2013 COMSTAC
