EVERYDAY INNOVATION OKINAWA’S PRELIMINARY RESULTS WITH 5 EXPANDED FUNCTION ENTERPRISES Everyday Innovations Fully implemented programs that have positively impacted healthcare delivery, improved operational efficiency or provided creative sustainable solutions to stubborn problems. Program History 3 MONTH SWOT ANALYSIS FOR 5 EXPANDED FUNCTION TEAMS • June 2002 expanded function trainers TAD’ed for 2 week course to Okinawa • Cost $ 4,300.00 travel dollar funds for 3 personnel(Hutto/Strothers/Munley) • Six teams trained( 6 doctors/18 technicians) • Two week (80 Hour training) for didactic/ hands on training • Create an environment focused on Teamwork and Outcomes UNCLASSIFIED
STRENGTHS # 1 3 MONTH SWOT ANALYSIS FOR 5 EXPANDED FUNCTION TEAMS • Increased access to care – average patients seen is120/month vs. 75/month. • Optimizing our technicians more effectively additionally inspires a motivational tool for subordinates. • Optimizes Dental Officers more efficiently. • Average DWV’s are 403/month (HANSEN) where single chair DWV’s is 190/month, over three-month period. STRENGTHS # 2 3 MONTH SWOT ANALYSIS FOR 5 EXPANDED FUNCTION TEAMS • Quality of care is the main focus; technicians take pride and ownership when placing high quality restorations. • Affords (patients) Marines the opportunity to complete comprehensive treatment – RDH, operative, oral surgery in one visit, minimizing the impact on Marines to complete their training missions vs. multiple appointments - great for UDP units. • Increased production by 38%, DHI increased by 5% - overall dental health has dramatically increased, (not health index)! WEAKNESSES # 1 3 MONTH SWOT ANALYSIS FOR 5 EXPANDED FUNCTION TEAMS • Trained dental technicians requesting leave or tasked for an operational commitment. • OPTAR for expanded functions dentist is the same as traditional one chair provider. • Training Practice Managers to efficiently optimize their resources – both tech and dental provider. In the past these Senior Enlisted Personnel were leaders, not managers – this is a total paradigm shift. UNCLASSIFIED
WEAKNESSES # 2 3 MONTH SWOT ANALYSIS FOR 5 EXPANDED FUNCTION TEAMS • Start up cost - required to standardize every room alike to increase efficiency. • Not enough “trained” technicians. UNCLASSIFIED
OPPORTUNITIES # 1 3 MONTH SWOT ANALYSIS FOR 5 EXPANDED FUNCTION TEAMS • “Run a Private Practice” within the Navy but keeping alignment of Navy Dentistry’s vision and mission. • Train all active technicians, hopefully at “A” School and decrease the need for contract positions - saving command money. • Additionally, more trained technicians may equate a rotationally basis for doc and technician alike, preventing “burnout”. THREATS # 1 3 MONTH SWOT ANALYSIS FOR 5 EXPANDED FUNCTION TEAMS • Lack of support from the directors. • Lack of support from Command Leadership. • Lack of support from the clinic as a whole, animosity buildup from those not expanded functions trained. • Providing additional requirements for the Expanded Functions Team – a computer, own phone system, own office space. THREATS # 2 3 MONTH SWOT ANALYSIS FOR 5 EXPANDED FUNCTION TEAMS • OPTAR, OPTAR, OPTAR – especially with the new Dental Classification system –(sealants are considered class II’s), if production is up, more consumables are required. • Loss of job security on behalf of the specialists. More chairs are required for E. F., taking away potential second treatment room from specialty care. UNCLASSIFIED

Observations3 MONTH SWOT ANALYSIS FOR 5 EXPANDED FUNCTION TEAMS Okinawa Program • Utilization of E-6’s as Program Managers seems be a important force multiplier. • Dental Officer’s feel they improve the quality of their preparations because they “..can take more time in preparation for caries removal and retention and resistance form even considering patient contact time is decreased by 60% for the Dentist since the Expanded Functions Technician will give 100% effort to the placement of the restoration.” UNCLASSIFIED
Observations3 MONTH SWOT ANALYSIS FOR 5 EXPANDED FUNCTION TEAMS Okinawa Program • Preliminary QA reviews have not identified any trends of substandard or iatrogenic events. • Ongoing QA review will continue. Consideration of one year follow-up at one year Annual Exam periods has been discussed. No plan of action has been implemented at this time.ECODS recommendation is pending. UNCLASSIFIED
