VSP Vision • Vision Service Plan (VSP) • Deductible None • Co-pay $0 • Exams Once every 12 months • Lenses Once every 12months • Single vision / lined bifocal / lined trifocal • Polycarbonate lenses for dependent children • Frames Once every 24 months • $150 allowance for a wide selection of frames • 20% off amount over your allowance • Contact Lenses Once every 12months • $130 allowance for contacts and the contact lens exam (fitting & evaluation) • 15% off contact lens exam (fitting and evaluation) • Glasses & Sunglasses • 20% off additional glasses & sunglasses, including lens options, from any VSP doctor within 12 months of your last WellVision Exam. • Laser Vision Correction • Average 15% off regular price or 5% off promotional price

Flexible Spending (Dependent Care) • Flex-Plan (www.flex-plan.com) • Dependent Care • $5,000 limit single ($5,000 married & filing jointly, or $2,500 if filing separately) • Coverage for dependent children, up to the age of 13 • Coverage for elderly parents needing daycare (living with you and a tax dependent) • Educational versus Custodial • Day Camp versus Overnight Camp

  7. Dependent Care Plan Details • This benefit operates on a plan year basis. The plan year is January 1st, through December 31st and all claimed expenses must have dates of service that fall within the plan year. • Claims must be filed by March 31st, 2014. • Money left in the plans at the end of the claims filing period cannot be refunded to you- these funds will be forfeited to Tintri

Dependent Care Reimbursement Process • Submit your claim to Flex-Plan using one of the following methods: • Fax – (425) 451-7002 or toll free (866) 535-9227 • Email – claims@flex-plan.com • Mail – Flex-Plan Services, PO Box 53250, Bellevue, WA 98015 • Cellular Phone (Android & iPhone apps = now available) • View Benefit Details • Submit Claims • Verify Benny Card Swipes • Get Account Alerts • Do not send originals. Keep copies of all documents submitted and fax or e-mail confirmations. • Allow 2 full business days for your claim to be processed. • Reimbursements will be issued according to the reimbursement schedule. • Direct deposits will be initiated and reimbursement checks sent on the scheduled reimbursement date.

Voluntary Life/AD&D • Mutual of Omaha • Voluntary Life (Post-tax employee-paid benefit via payroll deduction) • 5 x annual salary, up to an additional $500,000 • Guarantee Issue = $100,000 • Amounts exceeding the Guarantee Issue require completion of an Evidence of Insurability Form and additional coverage can be denied. • If you elect the minimum $10,000 benefit during this initial enrollment, you can elect an additional $10,000 during each subsequent Open Enrollment, up to the Guarantee Issue amount of $100,000. • Spouse – eligible for up to 100% of employee benefit, up to $50,000 • Guarantee Issue = 100% of employee benefit, up to $25,000 • Spouse rates are based on the age of the employee • Spouse coverage terminates at age 70 • Children – eligible for up to 100% of employee benefit, up to $10,000 • Guarantee Issue = 100% of employee benefit, up to $10,000 • Rates are not set and change at each age bracket, but only at Open Enrollment

Life & Disability Insurances • Group Life/AD&D Insurance (Employer-paid benefit) • 1 times annual salary, up to a maximum of $250,000 • Guarantee Issue = $250,000 • Voluntary Life/AD&D Insurance (Employee-paid option) NEW! • Employee = 5x salary, up to $500,000 (Guarantee Issue = $100,000) • Spouse = 100% of employee’s benefit, up to $50,000 (Guarantee Issue = $25,000) • Child(ren) = 100% of employee’s benefit, up to $10,000 (Guarantee Issue = $10,000) • Evidence of Insurability (EOI) required for any amount above Guarantee Issue • Portability & Conversion are available with no EOI required • Long-term Disability (LTD) – Employer-paid or Employee-paid (contributory) • 60% of before-tax monthly earnings, up to a maximum benefit of $10,000 per month • Post-tax contributory option (any benefit received would be tax-free) • 90-day Elimination Period (benefits duration to age 65)

BeneTrac (online enrollment) • BeneTrac (online enrollment system) https://www.eenroller.net/login.asp?ST=TNTR2929 You can access BeneTrac by going to the Filice Benefits Website: www.filice.com/benefits/tintri and selecting the BeneTrac tab • BeneTrac allows you the opportunity to see all of your benefit options and employee contributions throughout the year • You can make the following changes in BeneTrac during this Open Enrollment • Enroll in DENTAL and VISION, plus add or delete dependents • NOTE: If adding outside of Open Enrollment, you need a Qualifying Event • Enroll in Dependent Care Flexible Spending • Enroll in Voluntary Life for you, your spouse and/or child(ren) • Change your address, or any other information you find that is incorrect • Update your beneficiary information for Life/AD&D

  15. What do I need to do? • BeneTrac: • Log-in to BeneTrac (https://www.eenroller.net/login.asp?ST=CRSD1808) between Wednesday, 12-05 and Wednesday, 12-12 and make your selections. Instructions are provided. • OPEN ENROLLMENT CLOSES: Wednesday, December 12, 2012
