Professor Levent Türkeri from Istanbul (TR) was the winner of the urology challenge sessions which were chaired by that expert of audience participation and founder of this forum Prof. F. Debruyne (BE); in a very close run contest where all of the lectures given were of very high quality. In three state of the art lectures Prof. Türkeri comprehensively reviewed the problems which we face with tumour heterogeneity in the management of urothelial cancer: “Clearly we need improved markers for tumour behaviour, in order to most effectively individualise our therapy for patients in the future”. He emphasised the potential importance of a tertiary Gleason pattern in the management of prostate cancer and provide a well reasoned argument for the need for a paradigm shift in our treatment of naïve metastatic prostate cancer. He suggested a potential early role for chemotherapy in appropriately selected hormone resistant patients.
