ESRD Network Program • Established in 1978 by Congress • Currently 18 Networks • NW 14 is and most NW’s are non-profit organizations governed by volunteers from the renal community • 3-year contracts with Statement of Work defined by CMS • Funding from 50 cents withheld per treatment • www.esrdnetworks.org
Goals For the ESRD Networks • Improve quality and safety of dialysis related services • Improve independence, quality of life and rehabilitation (to the extent possible) through transplantation, use of self-care modalities, in-center self care through end of life • Improve collaboration with providers to ensure achievement of goals through most efficient and effective means possible, recognizing differences and associated possibilities/capabilities
Goals For the ESRD Networks • Improve collection, reliability, timeliness, and use of data to measure processes of care and outcomes; maintain patient registry • Improve patient perception of care and experience of care and resolve patient’s complaints and grievances
Mission Support quality dialysis and kidney transplant healthcare through patient services, education, quality improvement and data exchange.

How do I become a PAC member • Send a letter of interest to the Network office • AND • Have your facility Social Worker, your facility administrator, OR your Nephrologist send the Network office a letter of recommendation
