Guardianship Guardianship is recognized if: a) both parents are deceased b) both parents reside outside the contiguous United States and they are US citizens (usually military or job related) c) a student becomes the ward of the court and DSS assumes legal guardianship 1) In this case, the parents or former guardians retain no legal rights to or control over the student d) a student becomes the ward of an orphanage Please look at the sample forms on the screen
Reminders • All Special Form A’s must be typed. • If you are emailing, please send to jan@schsl.org or lavonda@schsl.org (not both) • Please fill in every blank that is applicable. • If it’s a parent to parent, please provide separation or divorce date. If never married, indicate that next to parent to parent move. • Don’t forget name of school and principal’s signature. • If your school is responsible for filling out the middle school forms, please remember 7th graders entering school for the first time do not need a transfer form. • The inter-district transfer paperwork for sub-varsity is only needed when there is no bona fide change of address. • Please call if you have any questions. 803-798-0120
