Summit Background • Thirty participant interdisciplinary consensus conference • Goals: • Determine minimum data elements to be included in a medication list; • Identify and discuss barriers and resolutions to the use of medication lists; and • Develop a framework for further research and a national social marketing campaign.
Immediate Next Steps • Publication of executive summary in the American Journal of Health-System Pharmacy • Development of a literacy-sensitive and patient-centered tool – My Medicine List • Promotion of My Medicine List during National Pharmacy Week (October 21 – 27, 2007) and National Poison Prevention Week (March 2008) • ASHP Foundation sponsorship of a University of Pittsburgh study entitled “Development of a Continuity of Care Record: Bridging the Medication Use Gap from Hospital to Home” • Collaboration with the National Transitions of Care Coalition • Inclusion in the American Pharmacists Association Medication Therapy Management Core Elements
