Introduction This module addresses the required Employee Registration Forms: • Employer/Employee Agreement • IRS Form W-4 • Department of Homeland Security Form I-9 Employee Eligibility Verification • Direct Deposit Form
CDC+ Employer/Employee Agreement • This agreement is between the employer and the employee, which outlines the employee’s: • Start date • Work schedule • Job duties • Hourly rate of pay • This agreement also establishes whether an employee is: • Exempt from employer taxes (Medicare, Social Security, and Federal and State Unemployment) • Eligible for overtime pay
CDC+ Employer/Employee Agreement Determines exemption from employer taxes (Medicare, social security, and federal and state unemployment) and worker eligibility for overtime pay.
IRS Form W-4 • An IRS W-4 form must be completed by the employee. • This form is completed so the correct federal income tax can be withheld from the employee’s pay.
IRS Form W-4 Determines the filing status and total number of allowances.
Department of Homeland Security Form I-9 Employment Eligibility Verification • The purpose of this form is for documentation that each new employee (both citizen and non-citizen) is authorized to work in the United States. • This form is required by the Department of Homeland Security for every employee.

Direct Deposit Form • The Direct Deposit Form authorizes the deposit of an employee paycheck or the consumer’s cash check into a checking or savings account. • An important advantage is that the pay is directly deposited and available on the pay day published in the payroll calendar. • Direct Deposit avoids loss or delay in the mail.
Employee Timesheet • A timesheet is required for every worker for each week of work. • The completed timesheet must be signed by the employee and the consumer or representative. • The CDC+ Program Office monitors consumer files and may ask to review timesheets for employees.
