  6. DIGNITY IN CARE GOLD STANDARD FRAMEWORK FOR CARE HOMES Kirklees 72% and Calderdale 63% of nursing homes will have completed GSF education by the end of this year Kirkwood Hospice support

DIGNITY IN CARE • GSFCH comments: • “we did palliative care well before but we do it even better now” • “before GSF residents were admitted into hospital to die now we have managed to care for all our residents in their terminal phase” • “we have developed a much better working relationship with our GPs since doing GSF” • “the staff are really motivated now they can see what a difference GSF has made”

  9. DIGNITY IN CARE • Integrated Care Pathway for the dying adult • Documentation complete • Ratification obtained in Kirklees • Piloting the document in GSF homes in Kirklees • Working towards Calderdale and other homes

  10. DIGNITY IN CARE • EDUCATION • Greater focus on in-house education • Numbers: 512 Attendance by Grade

  11. DIGNITY IN CARE • Training the Trainer – • 3 programmes currently ongoing • Role exchange – Kirkwood Hospice • 1st exchange complete – positive experience • 2nd to commence in the autumn • Clearer objectives, expectations and evaluation

  12. DIGNITY IN CARE • Co-ordinators course • Working with Leeds Metropolitan University and the GSFCH National Team • Criteria 3 levels: • Certificate 15 points • Diploma 30 points • Degree 60 points

  14. DIGNITY IN CARE The Future (1): • Embed the use of ICP within care homes • Support all the homes who have undertaken GSFCH programme keeping the gold gold • Engage further homes across this health economy onto GSFCH programmes

  15. DIGNITY IN CARE The Future (2) • Develop education – focusing on training the trainer and role exchange • Promote the Co-ordinators Course • Plan to publish articles around the role • Thanks to all who have support our work • Plan to take a well earned rest!
