  2. KY Consent • The KY Consent allows the dates of parental consent (for both Initial and Re- Evaluations), Date of Consent/Denial of Services, and when necessary, documents the reason for not meeting the 60 Day timeline requirement, to be collected in one document.

  3. Special Ed Evaluation Detail Report… • Is created using information collected from the KY Consent • Contains the information used to verify Indicator 11 compliance • If you do not use the KY Consent, you will have to review Initial Evaluations using the Record Review Document to report Indicator 11 compliance

  4. KY Consent • Path: Special Ed > General > Documents > New Document

  5. If a student does not qualify for services, place the date the ARC met to determine eligibility here From the drop down box, choose Initial or Reevaluation List the Primary Reason for missing the timeline as only one reason my be selected.

  6. Student Demographics Always click the Refresh Student Information button before saving. Demographic data will be populated/updated with Census data. Primary Disability auto populates from the student’s current, locked Conference Summary| Eligibility/Continued Eligibility editor, if applicable. (This is an initial, therefore no disability is yet identified).

  7. The information you select for each evaluation will show when printed. Don’t forget to list existing data!

  8. Only 1 disability is required but up to 3 maybe suspected Only check the sections chosen in the evaluations editor. Only select one option Now click Save & Continue

  9. Locking the KY Consent After you click Save & Continue, upon completing the Parental Consent, you automatically return to this screen. Click the Consent Complete box. It will look like this: Finally select Save

  10. Finding the KY Consent The KY Consent will be located in the Main School year folder, this one is 2014-2015, and then in the sub folder Conference Summaries/Evaluations/Consent
