  2. Medication Errors In Nursing Homes Ohio All nursing homes in Ohio must have a medication error rate below 5%. Minor medication errors below this number are generally acceptable by state standards, but all nursing home facilities must be free of significant medication errors. If the medication error rate for the facility is above 5%, the nursing home must create and implement a plan to correct these errors. Types Of Medication Errors In Ohio Nursing Homes: Slicing or cutting pills that should not be cut Not providing food or liquid to be taken with meds Failing to mix medication properly Improperly administering medication via IV or orally Borrowing medication for another patient Giving too much or too little of a medication Not diluting medication Skipping dose of medication Ignoring an order for medication Poor medication management Stealing medication Giving expired medications Your Rights in a Ohio Nursing Home Nursing home residents have rights under federal law, as well as under Ohio law. These rights not only help ensure that nursing homes guide their care with compassion, they also protect the residents’ dignity and privacy. When nursing homes and caregivers violate these rights, elderly residents can experience extreme suffering and harm. According to Medicare, nursing home residents have rights  that include, but are not limited to: The right to treat with dignity and respect, inform of services, inform of feeds prior to entering facility, privacy, keep personal belongings and property, refuse medications and treatments, a choice over your own schedule and activities, an environment that feels like home, inform about medical condition and medication. 21 N.
