NHS Context • From PCTs to NHSE & CCGs - NHSE specialist services and primary care contracting - CCG policy development with a local focus e.g. care homes • ‘5 Year Forward View’ (and other) - New models of care, including primary care - Reducing hospital admissions - Supporting people to remain living as independently as possible - Role of care homes

  10. Primary Care - Contractual Arrangements • Norwich Practices Limited (NPL) as contract holder with NHSE (separate dedicated patient list) • 18 month contract (as part of a pilot) • Use of CQC compliant treatment room • MoU between Norse and NPL • Revised specification between Norse and NCC

  11. Primary Care – Service Offer Demand-led, so still being finalised] • 5 day ‘working hours’ presence • Nurse practitioner service Mornings – surgeries Afternoons – home visits and Dementia Care Unit • GP clinics 2 days/week • On-site Community Pharmacist (working with Norse medication lead)
