Examples of Incidents/Incident History • Rings – electrical arcing, ripped fingers • 77 cuts/scraps to hands from 2004-2008 • Shoes – slips, trips, & falls is one of the top three incident categories (33 in 4 years) • Cut prevention – knife usage is an issue (3 in 4 years) DOW RESTRICTED - For internal use only

  5. HAO Knife Policy • Use of a knife to perform a task on any HAO site is prohibited • Authorize local variances for knife use only if a safer alternative tool cannot be found. If a knife variance is authorized, the task procedure must require the use of cut-resistant (Kevlar) gloves. • This policy does not pertain to the use of knives as eating utensils in designated kitchen/break rooms. HAO Site Clothing and Minimum PPE Policy • Houston Area Operations requires that all personnel including contactors and visitors wear pants or a skirt below the knee, sleeved shirt and closed toe and closed or strapped back shoes as a minimum for site entry. • Items not permitted to be worn on site while performing work: • Sleeveless shirt or tank top • Shorts of any type • Flip flops, crocs, open toe or open back footwear. • Heeled shoes greater than two inches high and/or less than one inch diameter. HAO Site Clothing and Minimum PPE Policy • Shorts and shirt can be worn, prior to changing into work clothes, when the designated change room is adjacent to a site entry access point or if a change room is located outside of a process area and accessible by personal vehicle. Work out attire, including shorts, can be worn in on-site fitness facilities. • Shorts are not allowed to be worn while riding bicycles, operating golf carts or company owned vehicles. • When wearing goggles, they are not to be attached to a hard hat or any other item that might cause the goggles to be blown or knocked from the eye protection position. • Goggle retainer straps/ rings will not be acceptable to carry goggles due to the manufactures limitations. HAO Glove Use Policy The Houston Area Operations Glove Use Policy requires that gloves must be worn 100% of the time by all personnel, contactors and visitors within all manufacturing process areas, maintenance and site logistics work areas. Cordoned or designated walk-ways/ thoroughfares in non-chemical process areas are exempt. This policy is intended to eliminate hand injuries and aid in the selection and use of gloves by all personnel. DOW RESTRICTED - For internal use only
