  2. Project purpose$33,523 Enhance communication, coordination and collaboration between the RAE (Region 1) and the counties involved in the West mountain regional health alliance (Pitkin, Garfield, Eagle and Summit) to improve member health outcomes

  3. THE PLAYERS

  4. Proposed process • Contract with WMRHA for development of regional priority work plan • Wmrha to conduct a webinar on the role, scope and priorities of the rae • Convene local county meeting with RAE to identify 1-3 county specific priorities, goals and outcome measures • Wmrha to conduct January 2019 regional stakeholders meeting to present county specific goals and facilitate creation of 1-3 priority goals for the region • WMRHA, counties and Rae work together to seek financial support and resources to implement the WMRHA/RAE action Plan

County specific priorities Pitkin • Develop a common community approach to reduce barriers to people entering the health care system • Improve Medicaid Benefit Literacy to increase utilization and access • Strengthen Care Coordination and Referral for Medicaid members Garfield • Improve Medicaid Benefit Literacy to increase utilization and access • Strengthen Care Coordination for Medicaid Members Eagle • Develop Behavioral Health System and Network with Total Health Alliance, WMRHA, RAE Partner • Improve navigation and education of the health care system • Strengthen Care Coordination for Medicaid members Summit • Rae Collaboration with state agencies (OIT, Peak, HCpf, Providers) around more transparent policies/processes for applicants and members • Create additional navigation and education for Members, providers and invested agencies

  6. 2019 Regional priorities 1.        Promote access and utilization of Medicaid benefits and Programs prioritizing Well Visits, annual dental visits and behavioral health engagement Objectives: -Develop Regional Communication MATERIALS TO PROMOTE THE 1,2,3 -Partner promotion of these benefits 2.        Strengthen Care Coordination System for Medicaid Members and Underinsured Objectives: -Follow up key points of care coordination survey 2017 (Survey Two: Completed June 2019), -Develop List of Best Practice Care Coordinator/Navigator Training around Medicaid and Programs. -Community Resource Network in 2020 will have shared case planning with clinics and all community resource providers 3.        Increase Medicaid provider enrollment Objectives: -Medicaid Provider List Kept Up-to-Date (Regional updates) -Secondary Focus County specific enrollment of all types of Medicaid providers (medical and dental)

Successes • WMRHA webinar – rae 101 for community partners • County specific meetings x 2 • Regional goals articulated and aligned with RAE deliverables • WMRHA approached by other West Slope Health Alliances who are interested in the process • Some hospitals included the county specific goals in the HTP • Partner survey reflects: • 65% satisfaction with county specific and regional goal setting • 64% satisfaction with Clarification of the Role of the RAE

Lessons learned • Clarification of roles, expectations and project purpose • Lack of depth in rural counties and other partners to coordinate and follow up • Contractor oversight and project champions • Rae participation • Data void • Goal setting- technical assistance to format • Multiple initiatives- PH plan + hs incentives + health alliances + hospital transformation + raedeliverables • Size of the region • Medicaid benefit Literacy • Communicate, Communicate, Communicate
