Health Application Overview • Designed for student screenings for pre-enrollment as well as new students. • Student rosters can be synchronized with the device prior to the screenings. • New students can be entered into the device with minimal data. • Device syncs with WiFi to upload data to WVEIS.

  3. Health App Requirements • WVEIS User ID for Web Based WVEIS • Including employee number • ‘Nurse’ job classification • WiFi connection within a county school facility.(Must be part of the “10.” education network and NOT a public IP address) • Download the application free through iTunes(Search for “WVEIS”)

  5. WVEIS Health Tab Entries from the iTouch Application or existing entries can be opened by clicking on the date. New entries can be manually entered by clicking on the ‘New’ link.

  6. WVEIS Health Tab • Information Stored: • BMI • Vision • Hearing • Oral Health • Developmental • Communication/Speech/Language

  7. WVEIS Health Tab - BMI Units of measure can be standard or metric. BMI is calculated by new entry or a change to the data. Note section can be used to store additional information.

  8. WVEIS Health Tab - Vision Referral source can be selected. Note section can be used to store additional information. Optional Data is still permitted for county use if desired.

  9. WVEIS Health Tab - Hearing Referral source can be selected. Note section can be used to store additional information. Optional Data is still permitted for county use if desired.

  10. WVEIS Health Tab – Oral Health Referral source can be selected. Note section can be used to store additional information.

  11. WVEIS Health Tab – Developmental Referral source can be selected. Note section can be used to store additional information.

  12. WVEIS Health Tab – Communication Referral source can be selected. Note section can be used to store additional information. Two additional fields are included for Articulation & Language.
