  4. Exercise – Part 3 (2 minutes) • Please share with one another what went well this past week and what you have chosen to work on for the coming week

  5. Improvement as a Path to PCMH • 10 FQHC look-alike clinics • Clinic staff meetings every other week • Plan-Do-Study-Act (PDSA) cycle as a vehicle • Regular coaching • Goals: engage front-line staff, foster innovation, standardize and spread, PCMH

Examples of PDSA Cycles • Time studies • Patient surveys • Pre-registration • Chart preparation • Morning huddles • Urine specimen cups at front desk • Faster prescription refill turnaround time • Improved handling of abnormal results • Tailored workflow by chief complaint • Improved medication reconciliation • Discharge checklists • Improved referral coordination • Laminated cards for staff communication • Walkie-talkies • Care teams by color • Walk-in clinic
