  1. Training Opportunity 2-28-07 What Happened? The incident occurred as the employee was placing 2x4 dunage under forms that were being off loaded by a skid steer loader. The employees hand was struck and pinned for an instant when the operator tilted the load downward. Upon a medical evaluation at a local clinic a fracture was found to the first metacarpal of his left hand. Left hand was struck between forms and 2x4.

  2. Training Opportunity 2-28-07 Corrective Measures Retrain Operator • The operator was trained to operate a bobcat. • Additional training will include proper communication techniques. Improve JHA • The Job Hazard Analysis was specific to the job task but improvements will be made to hazard recognition. Contributing Factors? Lack of Communication • The operator stated, “he was lowering the load so he could see the employee on the ground.” • The employee who was injured stated, “that their agreement was for him not to move the load until visual contact was made and communicated that all was clear.”
