Personal Portfolio • Students completing a health science cluster or program of study are encouraged to complete a portfolio. • The personal portfolio should be in narrative format to include: • information on the contents of the portfolio • any other relevant information of interest to the portfolio evaluators Foundation Standard 4: 4.4 Employability Preparation

Sample Projects • Plan and carry out a healthcare knowledge or skills application project that problem solving and hands-on application • Project should demonstrate a relationship to health science education or community healthcare issue • Project depicts the pros and cons of a healthcare issue Foundation Standard 4: 4.4 Employability Preparation

  7. Writing Sample • Demonstrates coherent thought • Follows accepted structural format for written work • Grammatically correct • Free of spelling errors Foundation Standard 4: 4.4 Employability Preparation

  8. Work-based Learning Documentation Documented by a summary of the: • exact hours • dates • person(s) shadowed • description of the experience Experience may include : • job shadowing • career exploration • internships • volunteering Foundation Standard 4: 4.4 Employability Preparation

Oral Report Must be a speech, presentation and/or instruction for: • school faculty • civic or community organization • local healthcare group • school classroom (any grade) • HOSA competitive event presentation • other health-related speech Foundation Standard 4: 4.4 Employability Preparation

Oral Report The report should include: • the script including who, where, what, and when the speech took place • an opening and a closing • the speech is cohesive and smoothly transitions between paragraphs • there are no detectable errors in grammar Foundation Standard 4: 4.4 Employability Preparation

  11. Service Learning/Community Service Must be documented by a summary of the experience to include: • dates • hours • location • learning outcomes • benefits to the community at large Foundation Standard 4: 4.4 Employability Preparation

Credentials School transcript or recent report card, and copies of certificates that show completion of training in health-related competencies Examples include but are not limited to: • CPR certification • First Aid certification • Certified Nurse Aide • Emergency Medical Technician • First Responder Foundation Standard 4: 4.4 Employability Preparation

Technology Skills Evidence of technology skills will be examples of technology created by the student • web site • electronic newsletter • PowerPoint presentation • other evidence of advanced technology skills certificate Evidence is detailed, error free and interesting Foundation Standard 4: 4.4 Employability Preparation

Leadership Examples • Leadership can be demonstrated by participation as a: • HOSA officer • class officer • student council • church officer • scout officer • other leadership position in the arts, athletics, or other organized school or community activities • List of principle leadership activities • Has held three or more offices or positions of leadership in the arts, athletics, and/or organized school or community activities Foundation Standard 4: 4.4 Employability Preparation
