CEDISPOSITIFSECOMPOSEDETROISCOORDINATIONS • Une coordination sociale avec une chargée de mission « aide à la vie quotidienne » sur chaque département • Une coordination des soins avec une infirmière coordinatrice en oncologie sur chaque département en lien avec chaque 3C et une infirmière coordinatrice en hématologie sur la région • Une coordination régionale avec un coordonnateur régional qui garanti la méthodologie régionale du dispositif

PUBLICCONCERNE • Ce dispositif s’adresse à toutes les personnes atteintes de cancer et leurs proches, en situation de difficultés dans leur parcours de soins soit par manque d’information, de compréhension ou de coordination dans celui-ci • L’infirmière coordinatrice intervient à toutes les phases de la maladie

  7. OBJECTIFSGENERAUX • S’assurer de la continuité et de la qualitédes soins entre les établissements de soins et le domicile • Garantir à chaque patient un parcours de soins personnalisé et efficace

OBJECTIFSSPECIFIQUES • Information/ Orientation • Informer les patients • Être le relais pour les établissements de soins, les professionnels libéraux, le malade et les proches • Faciliter la circulation d’information entre les différents acteurs de la prise en charge à domicile • Faciliter l’organisation du retour et du maintien à domicile • Évaluer les besoins • Rechercher une réponse rapide et adaptée • Mobiliser les ressources de soins • Renforcer les liens établissements de soins/ville
