01.01.01 Patient Identification • Use at least 2 pt. identifiers when providing care, treatment & services -Service includes giving blood, meds -Label all specimens in presence of pt. What 2 identifiers do you use in the hospital? What should you never use as an identifier?

01.03.01 Transfusion Errors • Eliminate transfusion errors r/t patient misidentification -What must be done before a pt. receives any blood product?
Eliminating transfusion errors: -verification by 2 people -matching component to the order and patient to the component
02.03.01 Report critical results of tests & diagnostic procedures on a timely basis Improve the Effectiveness of Communication Among Caregivers
What are critical results(definition)? • Who should receive the report? • What is the appropriate timeframe? • What should be done with the results?
Goal 3 Improve the Safety of Using Medications
03.04.01 Labeling Medications • Label all medications, medication containers, and other solutions on and off the sterile field in perioperative and other procedural settings.
Goal 3 Continued • Labeling Medications -name of med -strength -date & time -expiration
03.05.01 Anticoagulation Therapy • Reduce the likelihood of patient harm associated with the use of anticoagulant therapy.
GOAL 3 Continued • Use only unit dose prefilled or premixed bags when available • Each facility should have a protocol • The patient’s baseline coagulation status should be assessed
Goal 3 Continued • What tests should be performed prior to a person starting anticoagulation therapy? • Why should a IV pump be used? • What patient education should be reviewed with this patient?
03.06.01 Communication r/t Medication • Maintain and communicate accurate patient medication information.
