  4. Attendance Policy Revised 10/10 EAC The attendance policy of each instructor is to be submitted in writing to each student at the beginning of the course. Attendance is likely to affect the student’s achievement and it is at the instructor’s discretion as to how class attendance and participation are factored into the student’s overall grade for the course.

  5. Student Absences When a student must be away from campus for an extenuating circumstance, and particularly if longer than 3 days or more, notification for student absences will be issued by the Dean of Students Office, 715-232-1181 or email:  dean_of_students@uwstout.edu. Documentation is required. This notification does not change attendance policies put in effect by the instructor and is a courtesy memo only, letting the instructor know the whereabouts of the student. Using this process to determine excused absences and/or work assignments is determined by the instructor.

  6. New Add/Drop Procedures • Regular Session (semester courses) • Can ADD/DROP during first 7 calendar days or 5 business days on Access Stout. • Can DROP during first 14 calendar days via AccessStout. • ADDs after 7 days or drops after 14 days require a Program Card

  9. Other things to know Student employment records are part of their “educational records”. FERPA doesn’t apply to law enforcement records Heath and safety emergency exception is only for imminent threats Academic committee meetings in public forum may not discuss student academic records

UWS Ch. 14 Academic Misconduct • 14.03 • (a) seeks to claim credit for work or efforts of another w/o authorization or citation • (b) uses unauthorized materials or fabricated data in any academic exercise • (c) forges or falsifies academic documents or records • (d) intentionally impedes or damages the academic work of others • (e) engages in conduct aimed at making false representation of a student’s academic performance • (f) assists other students in any of these acts

I believe the following facts support my conclusion: (Description of the facts on which the instructor's conclusions were based.) In response to your actions in my course, I have decided upon the following sanction: (Specify in full.) As explained in section UW 14.06 (3) (c), you have the right to a hearing under s. UWS 14.08 to contest my determination that academic misconduct occurred, the choice of sanction, or both. If you desire a hearing, you must file a written request with the Dean of Students within ten days of personal delivery or mailing of this report. If you do not request a hearing within this period, the determination of academic misconduct shall be regarded as final, and the disciplinary sanction specified above shall be imposed. A copy of this report has been filed with the Dean of Students in order that the Dean may decide whether to seek further disciplinary action under Section 14.07 of the UW-Stout procedures. Questions on the process can be directed to the Dean of Students Office. Additional copies of the UW-Stout Academic Disciplinary Procedures are also available upon request. Grade Change Policy • Students can appeal a grade if: • Error in computation • Grading was based on factors other than those stated in the syllabus or that were prejudicial or capricious. • The student bears the burden of proof in establishing existence of above factors. • Appeal must be made within one year from time assigned
