ESSENTIALDUTIESANDRESPONSIBILITIESincludethefollowing. Otherdutiesmaybeassigned. ThePhysicalActivity & LifestyleSpecialist (PALS)willbe responsiblefordevelopinga relationshipwithproject participantstofacilitatetheintensivelifestyleinterventioncomponentofthePreventionofDiabetesProject grant forthe participants.Thiswillrequire aconsiderableinterpersonalskilllevel. ThePALSwillserveasafacilitator betweendiabetes programstaffandclient. ThePALSwillmaintainweeklycontactat aminimumwiththeirassignedparticipants intheactivephaseofthecurriculum andthen maintain regularcontactwiththemat regularintervals(notlessthanmonthly)tobespecifiedat alaterdatethroughout thecourseoftheprogram.Thesecontactswillencourageparticipation, answerquestions, facilitatebehavior change, anddevelopthe relationship requiredfor successinthisprogram. ThePALSwillworkclosely withtheproject participantsandfamilymemberstoidentify waystoincreaseexercise, andtoutilizeprogram“tools”tomeet exercisegoals. ThePALSwillhelpprojectparticipantsachievespecificweight lossbehavior changegoalsinastep-wisefashionusingstandardizedstrategies, trackingbooksandotherDPP“toolbox” strategies. ThePALSwillreinforcethe[educationcurriculum] lifestylebalancecore curriculum(developedout oftheDPP curriculum) withparticipantsandfamiliesastaughtby thepreventionprogramstaff.ThePALSwillfacilitateself-management using food recordsandphysicalactivityhabitlogs.The PALSwillassist increatinggroupactivitiestomaintainmotivationinthe intensivelifestylecomponent oftheproject.   2. ThePALSwillassistwiththemanagement, the “supportmaterials” componentsoftheproject forparticipants. ThePALS mayneedtotravelextensivelythroughout thetribalhealthserviceareatomeetwithparticipantstoachievethegoals stated, aswellastoparticipatein, plan, andimplement community-basedactivitiesandevents. ThePALSwilldocument allrelevant participantdataforthestudyparticipants. Assistsinthedevelopment andmonitoringofallperformanceimprovement initiatives and reporting requirementsofthe preventionprogram. Demonstratespositiveattitudeandprofessionalmanner, aswellasappropriateproblemsolvingandcommunications. Maintains cooperativeandeffectiveworking relationshipwithdiabetesprogramstaff, otherUIHSemployeesandsupervisorystaff, withtribalofficialsandvisitorstoUIHS. Meets allrequirementsofUIHS policy & procedure. Must haveawillingnesstolearn onthejob,employculturaltechniques, betrainedinsaidtechniquesas indicated. ThePALSwillagreetoparticipatein recruitment activitiesandlifestylesessionsthat mayoccurwithinoroutside regular workinghoursoronweekends. ThePALSwillbeanintegralpart ofamulti-disciplinary team(boththeDiabetesTeamandPreventionTeam), andwill workclosely withthediabetesprogramstafftoidentify impedimentstoadherenceand retentionintheproject.Theymay alsobeaskedtoparticipatein recruitment activitiesasneeded. Must bewillingtoparticipateinthelifestylebalanceprogramactivitiesinordertoserveasa mentorandexampletohelpfacilitate relationshipandbehavioralchangeforparticipantsintheintensiveprogram. Must beabletomaintainstrict confidenceofallinformation receivedonthejob. Participatesin mandatoryinitialtrainingandannualupdatetrainingasindicatedaswellas continuingeducationasrequired.
