  3. What Makes SFNP Unique? • Population of SFHS • Shared resources, programming, and data amongst stakeholders and CBOs • Shared leadership and accountability amongst all stakeholders

SFNP Model Grassroots and Collaborative Based • Goal: To increase number of students who stay in school, graduate, and are prepared for college or other postsecondary educational opportunities. • Achieve this Through: Holistically supporting educators, families and students by streamlining and networking current resources to meet needs and fill gaps. Not new programs, but new or refined methods • Our Community School Coordination Team acts as a stakeholder advocate and infrastructure to insure that services are relevant, streamlined, sustained--not shatter shot or silo’d--and have measurable positive impact. Integrated into school culture
