ACCIDENT DESCRIPTIONS • WHILE WALKING BACK INTO STORE. I STEPPED ON THE SIDEWALK AND MY HEAD HIT THE DRIVETHRU ROOF • PER FRIOI, SLIPPED ON WATER. WENT TO CATCH MYSELF AND INJURED MY ARM • PER TELEPHONE- I WAS WALKING THROUGH THE BAR, AND I WAS GOING TO UNLOAD THE TRUCK , AND I SLIPPED ON THE WET FLOOR AND I BLACKED OUT.

ONE OF THE GUYS PUSHED ME OUT OF THE WAY. `I WENT FLYING BACKWARDS, HIT A TABLE WITH MY BACK MOVED IT ALONG WITH A CHAIR AND FELL BACKWARDS TO THE FLOOR • WALKING TO FILL UP BEVERAGES IN THE BACK, SLIPPED AND FELL ON GREASE/WATER • WENT TO PUT A CRATE ON TOP OF A SHELF VASE FELL ON TOP OF MY HEAD.
