Surface Force Maintenance Effectiveness Reviews Cycles 23/25 Majority of Diving Systems reviewed MIP Series 5921, 5971, and H-012 1. Exceeded CNO reduction of 30% (over 39% actual) a. Deletion of redundant maintenance tasks. b. Age exploration of current tasks where possible. C. Shift towards “R” vice calendar periodicity's. 2. Periodic revisiting of systems for continued process improvement.
Reliability Centered Maintenance (RCM) A method for determining preventative maintenance requirements based on the analysis of the likely functional failures of hardware having a significant impact on safety, operations and support functions. 1. Initial development: Needs to happen in the planning stages with the ISEA, Design or Program Office and Sailors. A. Our maintenance program suffers due to the lack of this process not being enforced. There is no Waiver for the Diving Navy.
Reliability Centered Maintenance (RCM) cont. 1. Backfit Analysis: An analytical approach for the review of existing maintenance tasks to determine if the are in fact applicable and effective. 2. If the task does not meet the criteria, either modify it to do so or eliminate it. “Maintenance is designed to preserve function, not eliminate the risk of failure. You can minimize risk, but you cannot eliminate all failures”
Planned Maintenance • Sked 2.1 is available on www.ftscpac.navy.mil for download • Electronic FBR submission capability • Reduces response times • Allows for supervisors to track FBR from start to finish
Feedback Reports • Must contain UIC. • Be specific as to request (Tech manuals, MRC’s, MIP’s, etc) • Include email address, ph#, current POC. • Review applicable resources prior to submission (Sched Aids, Topside Tech Notes, 3M Manual, etc).

Current/Upcoming News Cont. • Maintenance for FADS III (Blowout disks) complete, will be issued in FR 2-01. • Overhaul of SCUBA regs extended to 24M-2R vice A-6R. • User calibration on gram scales for CO2 cartridges.
Failure Analysis Reports • If NAVSEA OOC Doesn’t know it is broke, then there is no way to fix it. • Electronic FAR submission on NAVSEA website. • Once received, they are forwarded to ISEA for review and resolution if possible. • If in DOUBT, report it.
Failure Analysis Reports Cont. • DSU FAR 001/01: ScubaPro Superhawk • Problem: Velcro fasteners are worn and have become unusable. Counter weight pouches have premature failure and are unusable. • Cost: 7000.00 (Replacement with Zeagle Rangers) • Mfg. Has been contacted and appraised of issues. Awtg response for corrective actions.
Failure Analysis Reports Cont. • EODMU-6 Det. Panama City: 083-001 • Problem: Pinhole leak discovered in shoulder of LUXFER 80cu ft. cylinder. • Cause: Cylinders mfg. in U.S. between 1972-1988 made of Aluminum alloy 6351. 2 cracks discovered using boroscope. • This material is subject to SLC (Sustained Load Cracking). Usually occurs in the neck or threaded portions. Difficult to detect at early stages with current inspection criteria/tools. • Post 1988 cylinders were made of 6061 alloy which LUXFER states is not susceptible to SLC. • Additional info on this issue at www.luxfer.com
Failure Analysis Reports Cont. • NSSF New London 002 • EODTU-2 Ft. Story • Problem: Failure of swaged fittings on Poseidon LP intermediate hose. • Cause: Possible Mfg. Defect in the production of hoses in the J/K lots made in 95/97. • Poseidon rep contacted, 3 hoses shipped to Sweden for inspection and testing. Replacement hoses shipped from Sweden. • Awtg amplifying guidance on possible replacement of these lots and or interim check to determine integrity of hoses.

The Future • Reduce FBR response time to coincide with OPNAV 4790.4C requirements. • Continue to streamline tasks and reduce man-hours where possible. • Consolidate/Upgrade HAZMAT items. • Add IEM tasks where applicable. • Provide timely and effective service to the Fleet, which remains the primary objective. Sailors are our most valuable asset.
