System Components • Sticker / Magnet Alert System for EMS • Emergency Health Information Sheet & Tube to House Sheet • On-line completion and updating of Emergency Health Information Sheet for parents on EMSC’s website www.health.utah.gov/ems/emsc • EMS Provider Education • Parent Education • Healthcare Provider Education • Promotion of program by School RNs so that a copy of the sheet will be kept at the child’s school
Who is eligible for this program? • Medical conditions that qualify a child for this program are: • Frequent seizures • Tracheostomy • Ventilator dependency- /CPAP/BIPAP • Congenital Heart Disease • Complex respiratory and cardiology needs • Neurologically compromised and requires suctioning for airway patency • Severe asthma with past admittance to ICU • Brittle Diabetics

EMS Provider Education • Key Information to Look For: • Name, birth date, primary language • Usual reason for calling EMS • Diagnoses • Procedures to avoid • Allergies • Baseline PE and vitals (weight, height, O2 Saturation) • Equipment and sizes • Best location for IV and blood draw • Medications and dosages • Common presenting problems

EMS Provider Education • When was the Emergency Health Information Sheet completed for a child? • Each page of the form is stamped on the bottom with the date it was last updated. • Parents are being asked to update the form EVERY time a medical change occurs for their child. • As EMS Providers, it is CRUCIAL that you ask any caregiver on scene if the information on the form is up to date.
EMS Provider Education How can I have families in my area with children that meet the program criteria enroll in the program? www.health.utah.gov/ems/emsc Once the form is completed, EMSC will mail out the alert sticker, magnet, and tube to the child’s caregiver
