  1. How To Fill Out The:USACC Form 104-R

  2. Block 6 (Grade Point Average) Complete for all college terms completed. Block 8 (Student Initials & Date) This is completed after initial 104-R has been completed during semester counseling / review. 104R – Page 1 Fill out all header info. DO NOT COMPLETE CIP Code Advisor should know, otherwise Mrs. DeWalt will complete MUST be the latest form. Earlier versions are not accepted.

  3. 104R – Page 2 Must fill out type of Degree, check “Yes” or “No”, and put Month/Year of graduation. Cadet must sign and date Advisor must sign and date

  4. 104R – Page 3 Fill out all info. Cadet must sign and date PMS sign and date

  5. Semester/Quarter Block Fill out “Term” (Fall, Spring or Summer), “Year”. Use 2 digits for the year. No. 1000 Course Title: PSYC Intro to Psychology Hrs. 3 Cts. 3 PSYC Intro to Psychology 3 1000 3 3 3 These fields auto calculate.
