Patient History • Patient is 78 years old. • Reports having poor dental work in the past. • Specifically remembers the handpiece bur hitting her tongue during a crown preparation that contained an amalgam build up. • Patient has positive history of Lichen Planus. • Medications: Carvedilol, Combivent, Lasix, Levothyroxine, Losartan
Lesion Description • The lesion is on the right posteriolateral side of the tongue. The lesion is adjacent to a tooth that contains an amalgam build up underneath a crown. The lesion is approximately 0.5cm horizontally and 2mm vertically with a couple of areas being more rounded. The lesion is of a darker bluish color and is well defined. It is asymptomatic and patient was unaware of the lesion.
Patient Dialogue • Has there been any change in the size or shape of the lesion? I don’t know • Have you noticed this blue lesion on the side of your tongue before? No • Are you taking any blood pressure medications or had a history of high blood pressure? Carvedilol for hypertension • Do you remember having any trauma to this area? Yes, when I was having a filling and crown prep the dentist hit my tongue with the bur. • Was the trauma at the same time of placement of a amalgam build up for your crown? Yes, he was putting a silver filling in under the crown.
Differential Diagnosis • Amalgam Tatoo • Varices
