Enteral Safety System www.NeoMedInc.com
Joint Commission Sentinel Event Alert • Issued to more than 12000 health care organizations • Avoid purchasing non-intravenous equipment with tubing connectors that permit connections with IV connectors • Asses risks of all tubing and take appropriate preventive measures • Avoid using adaptors to connect tubing and syringes • “The basic lesson from the reported cases of tubing and catheter misconnection error is that if it can happen it will happen” Dennis S. O’Leary, MD, President, Joint Commission www.NeoMedInc.com
Joint Commission Goal for 2006 Proposed Joint commission goals for 2006 require Hospitals to Identify and take action to minimize the risk of inappropriate clinical line connections. www.NeoMedInc.com
Converting to oral syringe delivery of medications and enteral formulas utilizing enteral only tubing eliminated the necessity for leur lock IV tubing and syringes, thereby reducing the potential for wrong route error. Neonatal Network ArticleJanuary 2006 www.NeoMedInc.com
American National Standards Institute Article • A standard was written in the American National Standards Institute about enteral feeding set connectors and adapters, The object of this article was to minimize the possibility of potentially harmful connections occurring between feeding sets and rigid leur connectors or other IV sets. It required that all adapters that provide a direct or functional connection of an enteral feeding set to a rigid female leur connector be eliminated. • The ultimate goal of this standard is the prevention of any reasonable likelihood of connections between enteral feeding sets and devices other than enteral access devices. www.NeoMedInc.com
Institute for safe medical Practices Article • In the article from ISMP nurses were extracting the medication from Nimodopine capsules per Mfg. labeling and injecting the contents with an 18 gauge needle via the NG tube and flushing with 30ml of normal saline, unfortunately in the chaos of the day the drug was drawn into a parenteral syringe and accidentally given IV resulting in death. • In light of this event the new protocol was changed for doses given via NG tube be administered with an amber oral dose syringe • Since this error has occurred numerous times with fatal results whenever like drugs are dispensed it is critical that the potential danger of accidental IV injection be communicated www.NeoMedInc.com
Silicone 30 Day Feeding Tubes • 5.0, 6.5, 8.0FR • 40, 60, 90CM Lengths • Connector Hubs come in Universal, Enteral Only, and Leur Locking. www.NeoMedInc.com
Orange Radiopaque stripe Silicone Material High density formulation 30 day dwell time Precision drilled holes Open distal end 3 different hub styles, enteral, leur locking and universal, 5,6.5,8 french sizes, 40,60,90cm lengths Identifies it as a feeding tube Ease of placement, non wetting surface, softer so baby can can suckle, no leaching of plasticizers No need for weights or stylet Lowers Hospital cost No sharp edges, less trauma Complete drainage, much less bacteria growth Silicone 30 day feeding tubes
Polyurethane 30 Day Feeding Tubes • 4.0, 5.0, 6.5, 8.0FR • 40, 60, 90CM Lengths • Connector Hubs come in Universal and Enteral Only www.NeoMedInc.com
Orange radiopaque stripe Larger bore size Open distal end Cheaper than silicone Identifies it as a feeding tube and show well on x-ray Higher flow rates, less clogging Allows for 30 day dwell time easy to aspirate 30 day Polyurethane Feeding tubes www.NeoMedInc.com
JOGNN Article • In a study in the Journal of Obstetric, Gynecological and Neonatal Nursing of Indwelling vs. Intermittent feeding tubes in Premature neonates, Of the two groups that were studied no statistical differences were found between the two groups, and that the intermittent method of feedings was more expensive www.NeoMedInc.com

Silicone requires a thicker walled catheter. As a result lower flow rates accompany smaller French sizes. Recommend shorter catheter lengths with the smaller French (5.0) sizes. Over time the patient should move from a 5 French to 6.5 to 8.0 to maximize flow rates Sizing the Feeding tube www.NeoMedInc.com
Securing feeding tubes • Securing the feeding tube can be done in a couple of ways: • Wrapping a piece of small tape completely around the tube and then placing another small piece of tape in a bow tie over that to anchor it • Using a securing device like Griplock www.NeoMedInc.com
Cleaning 30 day tubes • Should the feeding tube come out you can clean the feeding tube per hospital protocol (usually normal saline) and return after flushing www.NeoMedInc.com
