For physicians desiring MOC credit: • Physician providers in the practice will participate in monthly team practice meetings. Physicians will help set goals, implement changes, and measure improvements with the Practice Team to increase immunization rates. Practice Physician Leaders will keep attendance at monthly QI meetings. • Practice Team of Physician Leader, Office Nurse, Lead Nurse should attend both Learning Sessions and Every 4-6 Week Coaching Calls • Complete Learning Session Requirements including Office Immunization Survey and PDSA Cycles
Data Submission: • Practices in ImmPact2 will be able to run overall coverage reports on the four immunization measures monthly and then submit the data(num/den) in an electronic format by practice by 15th of each month. • For practices not in ImmPact2, for MOC credit, each physician must review 10 charts a month on children that are either age 2, 6, or 13. • Pediatric Hospitalists will review 10 random hospital charts from children discharged each month. • PDSA cycles results will be submitted monthly with data in order to compile ideas for Best Practices and Barriers that are arising
MOC requirements • Quality Counts will keep track of attendance for practice teams and data collection submission • Practice Leaders need to keep track of monthly meetings and office physicians participating in project • After final data submission in April, we will send out attestation forms that need to be signed by physicians and then physician leaders and then sent back to QC for Dr. Belisle to sign. QC will submit electronically to ABP and ABFM
