  4. Review of Data Collection • Chart reviews on 20 patients younger than 5 years submitted by the 15th of each month • Monthly progress reports submitted by the 30th of each month

  5. Feedback Reports • Summary of progress on goals • Includes data received by 15th of the month • Faxed or e-mailed to your office

  6. Conference Calls Purpose • Share successes and challenges. • Plan strategies to make improvements. • Provide toolkit feedback. • Share learning. • Get support. • Discuss new ideas. • Periodic clinical updates.

  7. Conference Calls • Who should attend • Improvement Team including physician lead • All interested staff • What to expect • Be prepared to report on progress, including cycles you have completed and what you learned. • Questions and guidance.

  9. Conference-Call Etiquette • Please keep background noise to a minimum. • Mute your speakerphone when you are not speaking. • Speak close to the speakerphone or use a handset. • Identify yourself before speaking.

  10. Conference-Call Etiquette • Speak one at a time and avoid interrupting. • Please do not call from a car or cell phone. • Be succinct. • Limit paper shuffling. • DO NOT use HOLD (background music).
