Purpose and format PURPOSE: Equip interested State leaders with common background knowledge (“getting on the same page”) and tools to pursue increased utilization of CHWs FORMAT: online PowerPoint slide shows with streaming audio narration Possible ongoing exchange for sharing ideas, experiences and resources: blog? conference calls? Tied to formal process evaluation

    4. Why the current interest in CHWs? Diversity of population and need for cultural competence Aging and growing prevalence of chronic diseases Growing complexity of health care services Recognition of importance of social and behavioral determinants Commitment to reducing health inequities Cost pressures on system Shortages of clinical personnel limiting time per patient

    5. Definition of CHW: American Public Health Association …a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the CHW to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. A CHW also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.
