Aging and dementia • a carepathhasbeendeveloped for the dementia client • the specialregionaldementianursestrainhome-careteamsto proceedaccording to thismodelusing as an example a realdemanding case of a client in the team • education • lessons to the personnel, for examplehow the encounter the demanding dementia client, stimulus activity for the dementia client, how to inform and collaborate with the relatives, how to notice and respect the autonomy of the client etc. • multiprofessionalmeetings with the physician of the team, dementianurse and with othercollaborators (e.g. geriatrician, social worker, physiotherapist) dealing with demandingclients in team As a part of a project to Cultivate promising practices for prevention of social exclusion of elderly people in the Helsinki home care system.   8. Activating Daily Living -model The individual care plan is complemented with a model of Activating Daily Living • which focuses on utilizing the individual`s strengths • encouraging spontaneous activity • enrichment of every day life with a ”Carpe diem” perspective

Taimi’sactivating daily living board • I like to be with other people (dancing, singing etc.) • I like to do housework • I exercise with Motomed twice a week (on Tuesday and on Thursday) • I love dogs and whenever there is dogvisitor in the house, I would like to be there • I love to spend time in Kustaankartano´s garden at least once a week
