  1. Broadspire TelePlus Claim Capture Online Intake Application

  2. The first step is to select the PEO that the claim is being reported for. Users will only have access to the PEO they are authorized for. The second step is for the user to enter their Name, Phone Number, Date of Loss and Time of Loss. As information is entered, the user must TAB through each field.

  3. A unique reference number is assigned for each loss reported. WELCOME screen will provide assistance with the application.

  4. Messages appear throughout the call flow to assist a user.

  5. The worksites related to the PEO will display for the particular state being entered. Either Highlight and Select one of the worksites or if the worksite is not displayed, select Quit to continue with the claim intake.

  6. Cedar Hill account Specific information is gathered on this screen

  7. The PEO information will prefill based on where the claim is being reported Each screen has a help button that will display a HELP page. The HELP page provides assistance for required entry.

  8. The loss location will prefill with the worksite address that was selected. If the loss occurred at a different address, the information can be updated. A zip code search is available that will populate the city and state fields.

  9. NCCI Job Classification search is available or manual entry is allowed. Additional Cedar Hill information is gathered throughout the claim intake.

  10. Injury coding is gathered based on standard industry NCCI codes. Fields will enable or disable. The date fields enable when the Loss Time field is equal to YES.

  11. Data entry assistance is displayed.

  12. Information regarding either the hospital or physician can be entered.

  13. Witness information is displayed.

  14. Data entry assistance is displayed.

  15. Additional comments can be sent to the claim handling office.. A copy of the claim acknowledgement will be sent via fax based on the number entered.

  16. Instructions for completing your loss are provided. Information regarding the Broadspire Claim Service Center will display at the end of the call.

  17. The Broadspire reference number is displayed when the loss is submitted.
