Freedom House provides integrated, person-centered behavioral health care for individuals and families with substance addiction or behavioral health needs. Services include screening, assessment, treatment planning, therapy, psychiatric evaluation, and referrals. Confidentiality and comprehensive discharge planning are ensured. Available to FCSS employees through the Employee Assistance Program.

  2. The Mission of Freedom House is to promote, enhance and support recovery for women, men children and families with behavioral health needs and /or substance addiction by using an integrated, person-centered approach.

  3. Employee Assistance Program • Available to all FCSS employees Services provided: • Screening and referral during business hours • Comprehensive Clinical Assessment • Treatment planning • Individual and group therapies • Psychiatric evaluation and med management • Referrals to additional services as needed

  4. How It Works FCSS employees may be referred by: • Human Resources • Supervisor • Self A screening and referral number will be staffed by a qualified mental health professional M-F 9A-4PM for employees needing to discuss their needs or get help making an appointment.

  5. Comprehensive Clinical Assessment Employees will be referred for a Comprehensive Clinical Assessment with a licensed clinician at Lake Area Health Care clinic in Warrenton. FCSS has agreed to cover the cost of this assessment for up to 50 employees in this first year.

  6. Treatment Planning Once the employee’s needs are determined by the assessment, the employee and clinicians will formulate a plan for treatment.

  7. Outpatient treatment • Employees will be referred to individual or group therapy depending on their needs and the availability of the service. • As needed, the employee may also be referred for a psychiatric evaluation and medication management. • Services will be provided as allowable by state insurance plan and employee will be responsible for deductibles and co-pays.

  8. Additional Services • Detox/Facility Based Crisis in Chapel Hill • 24 hour crisis services • Short-term stabilization • Evaluation, screening and triage • Individual, child and family services in Orange, Durham, Person, Warren counties • Individual and group therapy • Psychiatric evaluation and med management • Criminal Justice Services • Intensive In-home, In-Home Therapy Services, Peer Support, Integrated Primary Care • Services specifically for children, adolescents and their families • Halfway Houses in Chapel Hill, Durham and Norlina • Long-term residential treatment - 3-6 months

  9. Confidentiality Freedom House follows all applicable state and federal privacy laws in regard to consumer care. FCSS will work with the employee and Freedom House to gain consent for release of treatment information if needed.

  10. Discharge • Occurs when goals have been met, higher level of care needed or employee no longer desires service • Comprehensive discharge plan and referrals documented
