CASE PRESENTATION • A male patient admitted to A&E • Very agitated • Required restraint by both security and the police • Was having severe visual hallucinations.
Patient sedation was advised by mother • Patient tachycardiac. NRG-1, plant fertiliser, Naphthylpyrovalerone, energy-1, rave Naphyrone
Limited information available on effects Toxicity is similar to amphetamines In severe cases :- Stroke Myocardial infarction Hyponatraemia Rhabdomyolosis DIC Renal failure Hepatocellular damage Poor prognostic signs:- Hyperthermia seizures Focal neurological signs Coma
Nursing Care Plan • Maintain an adequate airway • Observe for 4 hours if asymptomatic • Pulse, blood pressure, body temperature. • 12 lead ECG • EC LFT and CK • Agitated adults require sedation • Correct any acidosis • Correct hypotension with IV • Nitrates and raising foot of bed • Correct hypothermia • If Ck raised due to rhabdomyolosis volume replacing alkalinisation to prevent or reduce rhabdomyolosis induced renal failure
