Highlights of 2012 Annual Open Enrollment: • There will be no changes to the Health Plan – rates will not change • There will be moderate increases in Dental rates • On-line enrollment option available again this year
TERMS YOU NEED TO KNOW….. • Deductible – The initial amount of medical or hospital expenses you must pay before the Plan starts paying benefits • Single Deductible – The amount that each covered person must pay during a calendar year before the plan begins paying benefits for that person. • Family Deductible – The maximum amount that two (2) or more family members covered under the plan must pay in a calendar year. Once this cumulative family deductible is reached, the deductible will be considered satisfied for all family members covered under the plan for the remainder of the calendar year. • Out-of-Pocket Max – The maximum dollar amount that you will have to pay for covered medical expenses during the plan year after which the plan pays 100% (the annual deductible counts toward the annual OOP, Office Visit or Rx co-pays do not) • Co-Insurance – A percentage of the eligible expenses that you are responsible to pay the hospital or other provider/facility after meeting your annual deductible • Co-pay – A fixed dollar amount that you pay when you use services, such as doctor visits and prescriptions. Co-payments are not subject to the annual deductible.
Other Benefit Plans • Dental – Moderate rate increases • Life Insurance • Enhancement! - The university provided basic life insurance amount will increase from $15,000 to $30,000 effective January 1, 2012. • No action needed during open enrollment for this benefit change • No rate changes for optional/spousal life insurance • UNLESS….you or a spouse progress to a new age band • Remember to keep beneficiary information up to date • Short Term Disability – No changes • UNLESS….you progress to a new age band or experience a salary increase
Flexible Spending Accounts • Voluntary Medical and Dependent Care FSA’s REQUIRE annual re-enrollment • Employees who waive/opt out of the WKU Health Plan DO NOT need to re-enroll this year • New debit cards WILL NOT be issued to current participants (DO NOT THROW AWAY YOUR CARDS!) • Existing cards will be re-loaded with new election amounts • REMEMBER….Over-the-counter medications are no longer eligible for reimbursement (unless prescribed by a physician)
How to Enroll • Attend Open Enrollment on Nov. 8 & 9 to complete an enrollment form • Benefits Fair at the Kentucky Building • All benefit and retirement vendors will be on-site • Enroll On-line and submit electronically using e-signature forms. On-line form will be available on HR Benefit website • Download the enrollment form from the HR Benefits website, complete, and return to WAB, HR, Room G25, Attn: Benefits
On-line Enrollment Form • This year the on-line e-signature form will be pre-populated with your current benefit elections • You should only make changes/edit the form as needed • The life insurance beneficiary information is not maintained electronically; therefore it will not pre-populate on the enrollment form. Update on the form as needed.
