  1. How does Feedback Form 6B work? for Appraisers (Primary and Secondary Care)

Feedback Forms 6B • There are three forms to be completed after the appraisal: • Form 4 (Summary) • Form 6A: Appraisee Feedback • Form 6B: Appraiser Feedback • Prior to 1st April 2014, Feedback Forms were accessed and processed on an external web service – this has since been reintroduced as part of SOAR • This guide focuses on the reintroduced Feedback Forms on SOAR • Previous data collected will be inputted into SOAR in due course

How the Feedback Forms work (1/2) • Feedback Forms will automatically be made available on the interview page – for both Appraisee (6A) and Appraiser (6B) from: • the date of interview (as detailed on SOAR); and • when Form 4 has been created by the Appraiser • Feedback Forms will be available for 8 weeks, after which they will be closed automatically • SOAR will send out automated email reminders at 6 and 7-week intervals (if not completed already)

How the Feedback Forms work (2/2) • Appraisees will be asked for their feedback automatically after every appraisal • Appraisers will be asked for their feedback automatically for the first 10 appraisals • After which, completion of Form 6B is optional for Appraisers • For example, you may wish to use the feedback form to reflect on a particularly difficult appraisal

  5. First 10x Appraisals, Feedback Form 6B will automatically appear for Appraisers, if: • Interview date is reached; and • Form 4 has been created • Thereafter, it is optional via button • Feedback Form automatically closes after 8 weeks • Can edit, if needed, within this 8 week period

  6. Questions on Feedback Forms for first 10x appraisals are based more on ratings • Simple to complete – click Submit when finished • When submitted, Feedback form is locked • Questions on feedback forms thereafter are more reflective / allow free text comments

  7. How the feedback is used • GP Appraisers are required to undergo annual performance reviews with their Local Appraisal Adviser. System-collated Feedback reports are provided to both for discussion • Secondary Care Appraisers may wish to review and reflect on the feedback received • Local Appraisal Advisers / Leads (and NES) can review the collated (and anonymised) Feedback reports for improving local processes • NES uses the comparable data for annual reports • None of the feedback reports are shared with 3rd party agencies
