PURPOSE To describe the incidence of LASIK complications related with microkeratomes
METHODS • Retrospective, comparative, interventional case series. • We reviewed the records of patients who underwent LASIK at Excimer Laser Santa Cruz, São Paulo, Brazil (65503 eyes). • Type of complication,microkeratome and keratometric readingswere studied.
RESULTSType of Complication • 380 eyes (0.0058%) with an intraoperative complication were identified. • Complications included incomplete flaps (0.21%), buttonholes (0.09%), thin flaps (0.06%), and free flaps (0.07%).
RESULTSMicrokeratome • The Hansatome was the most common microkeratome used (44.884 eyes; 68,52%), followed bythe Automated CornealShaper-ACS (14.652 eyes, 22,36%) and the Nidek MK-2000 (5967 eyes, 9,1%).
RESULTSMicrokeratome • Intraoperative complications were more common with the ACS (1.31%) than with the Hansatome (0.62%) and Nidek MK-2000 (0.64%) (P<.001)
RESULTSMicrokeratome • Buttonholes were more frequent with the ACS (0.32%) than with the Hansatome (0.07%) (P<.001) and free flaps were more common with the ACS (0.27%) and MK-2000 (0.3%) than with the Hansatome (0.05%) (P<.001).
CONCLUSIONS • Complications with mechanical microkeratomes to perform LASIK surgery are extremely rare • Most of the complications involve construction of corneal flap • There is no significant disadvantage compared with the femtonsecond laser • Intraoperative complications were more common with the ACS than with the Hansatome or MK-2000. Buttonholes were most frequent with the ACS, and the Hansatome had the lowest incidence of free flaps
