What is SNIAC? A mandatory program for all active duty service members who have a family member with special medical, educational, or behavioral needs - This program assists Air Force Personnel Center (AFPC) in assigning sponsors to areas where family members’ medical or educational needs can be accommodated
Enrollment in SNIAC • A diagnosis of ADD/ADHD • Requires special medical or adaptive equipment (i.e. nebulizer, wheelchair, home oxygen therapy, etc.) • Has developmental delay or requires special education, as specified on IEP (Individualized Education Plan) or IFSP (Individualized Family Service Plan) • See 2792-1, page 3, block 4.a for a list of services
DD FORM 2792-1 Page 2 is completed by Air Force member or spouse (parents of student)
DD FORM 2792-1 (cont.) Page 3, items 1-2, are to be completed by parents. Item 1.a. is release of information from parent to school. Page 3, items 3 -6, are to be completed by school or early intervention staff. If a child does not receive services mark (x) No in blocks 3.a.-e., complete block 6 (school information) and return to parent.
DD FORM 2792-1 (cont.) If yes is marked in items 3.b. or c., remainder of form must be completed. *please attach current IFSP or IEP Item 4.a.-b. Eligibility Criteria. Mark only one. (Codes in 4.a. are for Army coding only.) Item 4.c. Identify the disability, if known. (For example, blindness, autism, PDD.) Item 5. Severity. Mark only one.
Common Concerns The Form is a Department of Defense form and is not easy to follow? • The school is responsible for completing only page 3 of the DD FORM 2792-1. The form is currently under revision and a more user friendly version is set for release in Nov 2009. The child is home schooled or between the ages of 3-5. “Not enrolled in the school” • The Individuals with Disability Education Act (IDEA) entitles that these children are eligible for special education services through their local school. If the child has never been evaluated , is not currently receiving services, and/or has no identified needs that the school has been made aware of…there is no liability attached. The form can be completed by the school. • If the child does not receive services and has no special needs identified, the school can elect to leave the form blank and simply write a letter (on official letterhead) stating that : “named child has no identified special needs and is not receiving services at this time”. Air Force preference would be that the school official completes the DD FORM 2792-1 and marks no to all responses on item 3, skips to item 6, and returns form to parent.
