Methodology & Background • The Patient Navigation Center served 3,609 unique individuals during its first eleven months of operation • Our team of 4 Patient Navigators assisted patients through 9,525 interactions, comprising phone calls, emails, texts, website live chats, and Skype calls. • The Patient Navigation Center caseload has grown approximately 420% since opening in February 2016 • Health education inquiries have grown over 500% compared to the same time period during the previous year (2015)

Stories from the Navigation Center • David • Living with psoriasis • Not satisfied with his health care provider and not treating • Having trouble understanding what treatment options were available to him and how his insurance would cover the treatment(s) • In working with his Navigator, we were able to: • Help David find a new specialist to treat his psoriasis • Help him understand how to purchase a home phototherapy unit through his insurance, which is now adequately addressing his disease • Provide coaching to help him achieve his goals for overall wellness and stress-reduction

Stories from the Navigation Center • Stephen • Has had psoriasis for most of his adult life • Had only treated with over-the-counter topicals • Was feeling isolated, hopeless and unsure of where to turn next • In working with his Navigator, we were able to: • Help Stephen locate a qualified dermatologist • Educate Stephen about prescription options for psoriasis treatment • Provide resources to help Stephen better understand his psoriasis and connect with others • Stephen now uses light therapy to manage his condition. He is optimistic about his other options should he ever need to change his treatment plan.

Stories from the Navigation Center • Ryan • Has severe and degenerative psoriatic arthritis • Living in a skilled nursing facility • Has other comorbid conditions which make managing his psoriatic arthritis more difficult • In working with his Navigator, we were able to: • Help Ryan enroll in a vocational rehab program, where he can pursue a degree through online learning and eventually find long-term employment • Connect him with a psoriatic arthritis specialist who is able to make visits to his nursing facility • Ryan finally feels optimistic about his treatment plan and future with psoriatic arthritis.

Stories from the Navigation Center • Eileen • Son has severe psoriasis • Fear of side effects kept them from seeking care • In working with her Navigator, we were able to: • Connect Eileen’s son with a pediatric dermatologist • Provide them with information about treatment options and resources to facilitate a good conversation with the specialist • Connect Eileen and her son with other families impacted by psoriatic disease • Eileen’s son started a biologic treatment and he is seeing clearance. The Navigation Center continues to support Eileen and her son through access to care challenges and other obstacles.

Stories from the Navigation Center • Carol • Has severe psoriasis • Was prescribed a systemic treatment by her dermatologist but contacted the Navigation Center due to fear of side effects • In working with her Navigator, we were able to: • Share more information with Carol about the relative risk of the treatment, as well as its benefits • Help her feel more comfortable sharing her concerns with her health care provider • After working with the Navigation Center, Carol felt much more informed and empowered. She started the treatment prescribed by her health care provider and is now doing well.

Stories from the Navigation Center • Shelley • Has severe psoriasis • Not currently working due to the severity of her disease • Uninsured and unable to afford treatment • Through working with her Navigator, we were able to: • Help Shelley understand her plan options on her state exchange • Explain insurance terminology and how to best utilize her coverage • Connect her with an in-person enrollment counselor for further support and resources • Shelley now has insurance through the exchange and is looking forward to being able to treat her psoriasis again. She also wants to share her story with others and be a voice for the importance of access to treatment.

Stories from the Navigation Center • Donna • On a biologic treatment • Has commercial insurance but will be transitioning to Medicare next year • Seeking information and resources to help keep her treatment affordable • In working with her Navigator, we were able to: • Help Donna understand the components of her future Medicare coverage • Advise on a plan that has her health care providers in network, all of her prescriptions on the formulary and will keep her costs low • Donna has selected a plan and feels much more confident in her understanding of insurance terminology and ability to advocate for herself

Stories from the Navigation Center • Barbara • Has Medicare coverage • Taking a biologic for her psoriasis • Faced with extremely high out-of-pocket costs • In working with her Navigator, we were able to: • Help her understand her Medicare benefits and the most advantageous way to bill for her treatments • Offer enrollment information about assistance programs that can help lower her costs for treatment • Provide guidance to explore other plan options during Medicare open enrollment • With the information she received from the Patient Navigation Center, Barbara was able to afford her treatment and continues to see great results

Stories from the Navigation Center • Pablo • Has severe psoriasis • Initially contacted the Navigation Center for information about biologic treatments • Decided on a treatment with his health care provider, which was then denied by his insurance • In working with his Navigator, we were able to: • Help Pablo collect paperwork on his insurance denial • Appeal the denial in partnership with his health care provider • Connect him with the pharmaceutical hub for further resources and a bridge supply of the medication • Pablo is now on the biologic treatment and doing very well

Stories from the Navigation Center • Erick • Reached out to the Navigation Center for treatment information, help finding a provider, and assistance applying for disability • Very limited insurance (due to immigrant status) and financial resources for treatment • Could not afford any of the medications his doctor prescribed • In working with his Navigator, we were able to: • Help him apply for full-scope Medicaid, for which he was approved • Locate a qualified health care provider who accepts his insurance • Help him secure his prescriptions at low costs • Now Erick is finally on a treatment plan which is controlling his psoriasis symptoms
