Case Report: Pang Meng-liang潘夢良 • Age:12 • Vision: Finger counting • Help from brothers and classmates • cooking lunch and going to toilet • Teachers very concern • Going blind but why?
Examination with the ophthalmoscope • cataract (both eyes) • Teachers • parents said girl was very sick a few years back • Not sure, but vision got worse after recovery from sickness (?)
Cataract Risk factors • Everyone is at risk of developing cataracts • age is the single greatest risk factor • By age 65 about half of all Americans have developed some degree of lens clouding, although vision may not be significantly impaired
Other factors that increase risk of cataracts include: • Diabetes • Family history of cataracts • Previous eye injury or inflammation • Previous eye surgery • Prolonged use of corticosteroids • Excessive exposure to sunlight • Exposure to ionizing radiation • Smoking
What causes cataracts in children? • Can be congenital or may develop during the first few months/years of life • Inherited or related to metabolic or systemic abnormalities • Many cases, cause unknown • IF visually significant cataract occurs in a child, prompt surgery and rehabilitation required to maximize visual development (cataract does not allow the eyes to get proper visual stimulus to develop)
visual outcome for children with cataracts is much more optimistic now than it was a few years ago • However, if associated with abnormalities limit visual potential that can be obtained, despite early surgery and adequate visual rehabilitation
Treatment • cataracts can be remedied by relatively simple surgery • operation generally lasts only about 15 minutes, usually performed on an outpatient basis If left untreated…. • Glaucoma is a potential complication
The case of Meng Liang夢良… • Mr Wei (Assistant Minister of UF) • Free cataract surgery available in big towns eg. Nanning • ??? • Emailed Mr Li • Arrange for Meng Liang夢良 to have surgery ASAP • Winnie to follow up
(Email from Winnie) • Mr Li • Follow up • Long queue (over 200) for free cataract surgery • will sent report…..
