Wisconsin Stroke Coalition Mission The mission of the Wisconsin Stroke Coalition (WSC) is to reduce the burden of stroke in Wisconsin, improve patient outcomes and support a healthy population through stroke prevention.
Participants • The WSC is open to organizations, hospitals, healthcare providers and stroke survivors who share the commitment to improving the quality of stroke patients’ care and care transitions.
Proposed Committees 2013-2015 Steering Committee (Chair, Coverdell Program Staff, EMS Unit, AHA/ASA) Rehabilitation Prevention Stroke Systems of Care (and Telestroke) Data/Performance Improvement
WSC Proposed Priorities 2013-2015 • Stimulate communication between stroke organizations in Wisconsin • Coordinate activities that can help fulfill the WSC mission • Establish stroke systems of care • Communicate progress being made • Plan for sustainability
Committee Structure • WSC meets quarterly – March, June, September, December • Chairs needed for each committee; meet as appropriate • Conference line available through DHS
Project Charter • Highlights groups’ purpose and goals • Includes milestones and general timeline • Identifies project sponsors and committee responsibilities • Demonstrates shared expectations and focus
