Background & History… • Summer 2017 - ‘Encouraged’ to follow Larwood’sPrimary Care Home model • Initially just a meeting of GP’s from ‘traditional’ Primary Care • Reluctance to form - Difficulty appointing Chair • Historic Context of failed “Retford Health” – a Primary Care collaboration to setup a Provider Arm under Practice Based Commissioning

  3. 5 Practices • List size range~ 17k patients • All CQC rated ‘Good’ • Currently no significant recruitment difficulties • All Performing well independently in challenging conditions

Wider Engagement Tuxford Medical Centre North Leverton Surgery Kingfisher Family Practice Crown House Surgery Riverside Health Talkzone Integrated Care Partnership Bassetlaw Action Centre Bassetlaw CCG Notts County Council Bassetlaw Community Voluntary Service (BCVS) Notts Community Healthcare Trust Retford Oaks Academy Patient Participation Job Centre Working Win

  5. Organisational Achievements … • Quickly built up a large Network of wider Services & Providers • Formation of ‘executive’ team to move things forwards between meetings. • Change of meeting setup – 90 minutes with ‘everyone’ split into 30 minutes of Primary Care and 60 minutes with ‘everyone’

  6. Tangible Achievements… • Setup Pain Management Self Help Group • Teen Counselling Service • School ‘NHS Careers’ event at Retford Oaks – well received by both students & parents • Starting to evaluate ‘Working at Scale’ opportunities more thoroughly

  7. Tangible Achievements… • Extended Hours Hub set up on PCN Footprint • Hub now also give us ability to develop other shared services • Idea-o-meter • Starting to consider Community Health Promotion in conjunction with Commerce John Adair ‘Lobster Pot’
