In respect of clients trading in derivative segment, the member shall collect documentary evidence for financial information. • Illustrative list of documents • Copy of ITR Acknowledgement • Copy of Annual Accounts • Copy of Form 16 in case of salary • income • Net worth certificate • Salary Slip • Bank account statement for last 6 • months • Copy of demat account Holding • statement. • Any other relevant documents • substantiating ownership of assets. • Self declaration along with relevant • supporting.  Provide client remarks , if any

  9. As per SEBI directive DP has to record mandatorily nomination for all new accounts. Account holder has to sign the nomination page with declaration Yes or No for nomination.
